Flu Information 2025-2026

If you received your flu shot from Employee Health:

Go into your ReadySet account and screenshot your flu vaccine information. Upload this into your Baptist Works LEAP portal. If you don’t know where to find the flu vaccine in ReadySet, click the button below to see screenshots of where to access the information.



IMPORTANT: Flu documentation MUST be uploaded into your Baptist Works portal (which is different than ReadySet)

Flu vaccines are required to be received by October 31, 2025! If we have not received your vaccine proof by 10/31, RMC will be required to remove you from the schedule.

In order to count for the 2025-2026 flu season, your flu shot must have been administered NO EARLIER THAN September 2025.

Getting your flu shot somewhere other than Employee Health?

This form is the ONLY document accepted by Employee Health if you get your flu shot outside of Baptist Health. Download this form and upload it to your Baptist Works Portal.

Religious & Medical Exemptions

For medical exemptions, please reach out to Whitney or Kristy with instructions.

All medical and religious exemptions must be sent to Employee Health by October 1, 2025 to be considered.

Where can I get the flu shot at Baptist?

Employee Health will provide flu vaccine clinics at all Baptist Health hospitals. You’ll also have access to flu vaccines on your unit- just speak with your Unit Manager, Charge Nurse, or Clinical Pharmacist. You can also check out the Employee Health Engage page for more info.

How to upload your flu documentation into your Baptist Works portal:

Access Flu Vaccine Information in ReadySet

The last screenshot is what needs to be saved and sent to Baptist Works. Please make sure your name and DOB are included.

Frequently Asked Questions

Can I get the vaccine at a pharmacy?

Yes, but you must provide the Flu Received Elsewhere document with the information filled out and signed by the person who administered the vaccine. You can download that form HERE.

Can I decline the flu vaccine?

No. Baptist Health requires that all staff members receive the flu vaccine. If you would like to request medical or religious exemption, you can do so by following the instructions listed above.

If I get the vaccine with Employee Health, does Baptist Works need a copy?

Yes! Please upload your proof of vaccination into your Baptist Works Employee Portal.

Is the Baptist Works Portal the same as ReadySet?

No! Your Baptist Works Portal (also called LEAP) is specifically for Baptist Works. Think of this like any other travel agency you’ve worked with previously. ReadySet is the website where Baptist Health’s Employee Health team keeps their documentation. The Baptist Works team does not have access to your ReadySet account.
